Print one person’s health details in a single PDF and choose which profile fields appear on the report.
Health Report
The Health Report is a printable PDF that puts one person’s health details in one place. Use it when you need a paper copy or a single file to review, share, or keep on hand.
It can include:

Basic profile details

-
Health questions you choose

Insurance details

Medications
Open the Health Report from a profile
Use the person’s profile to download the latest version of the report.
Open the person’s profile
Go to the person’s profile.
Download the report
Select Actions, then choose Download health report (PDF).
Make a field show on the report
Turn on the report setting for any profile field you want to include.
Open profile fields
Go to Admin > Profile Fields.
Edit the field
Find the field you want, then open it to edit.
Include it on the report
Turn on Include on health report, then click Update Profile Field.
If you are creating a new field, turn on Include on health report before you save it.
The report shows the latest saved version of the field settings. If you just changed a field, download the PDF again to refresh what you see.
If a field still does not appear
Check the most common causes before you try again.
Confirm you are viewing the right user type
Open the field settings and make sure the field is meant for the type of person you are viewing, such as camper or staff.
Check whether the answer is filled in
If the field has no answer yet, the report may show a blank line instead of a value.
Download a new copy
Save your changes, then download the PDF again so you are looking at the latest version.
If the field is still missing after these checks, review the field settings once more and confirm that Include on health report is turned on.
